STEP 1:

 

TEAM EVENTS:  Each team must complete an ENTRY FORM

INDIVIDUAL TRAINING:  Each player must complete a REGISTRATION FORM

 

STEP 2:

 

ALL PLAYERS MUST sign a WAIVER of LIABILITY  FORM  before participating.  

 

STEP 3:

The Entry or Registration form, and PAYMENT, and WAIVER FORMS must be received by DAP SPORTS 5 days prior to the event.     Mail forms and fees to:
